PRECAUTIONS FOR OUR PATIENTS

please read the following guidelines to help us during this pandemic

before your appointment...

A staff member will contact you a few days before your appointment. Please be ready to answer the following questions. If you are unavailable at the time of our attempts to contact you, these questions will be reiterated at the time of your appointment. 

HAVE YOU... 

1) Had a fever of 100.0°F or greater?
2) Had any recent travel? 
3) Had any exposure to someone who is sick?
4) Had any history of COVID-19? 

ARE YOU EXPERIENCING... 

a. A fever or shaking chills? 
b. Cough?
c. Shortness of breath? 
d. Nausea or diarrhea? 
e. Headache? 
f. Sore throat? 
g. Loss of smell or taste? 
before you enter for your visit... 

REMEMBER TO... 

** Masks are recommended, but no longer required to enter our office. We ask that our patients make their best judgement.

**While we are easing up on COVID regulations at our office, we ask for your continued patience as we navigate our way back to standard practices. 

once you are inside...

KEEP IN MIND

1. To avoid blocked off areas or chairs, and to practice social distancing. 
2. Reduce the amount of contact with other patients as much as possible. 
3. To inform a staff member if you have come into contact with any surfaces, or merchandise without supervision. 

WHEN SHOPPING

1. A lab technician will be assigned to supervise your shopping experience. 
2. To encourage social distancing, and ensure proper sanitation, only one shopper may be permitted to a shopping area at a time. 
3. If the shopping area becomes too congested, you may be asked to schedule your shopping to another date.
